Protective effect of dexmedetomidine on Tourniquet induced lung injury in patients undergoing total knee arthroplasty: A randomized trial

نویسندگان

چکیده

Objective To investigate whether dexmedetomidine (Dex) can reduce the severity of tourniquet-induced lung injury. Methods 36 patients undergoing total knee arthroplasty with a tourniquet were randomly assigned to control (ischemia/reperfusion [I/R]) group and Dex group. Patients in received loading dose (0.8 μg/kg over 10 min intravenously) followed by continuous infusion (0.5 μg/kg/h until end surgery. The I/R an equal amount 0.9% saline instead Dex. serum concentrations tumor necrosis factor-α (TNF-α), Clara cell protein (CC-16), soluble receptor for advanced glycation products (sRAGE), brain-derived neurotrophic factor (BDNF) measured arterial blood gas analysis was performed before anesthesia 30 min, 6 h, 24 h after release. Results In group, compared baseline, TNF-α, CC-16, sRAGE higher ( p < 0.05) BDNF concentration lower at most time points. than those 0.05), whereas 0.05). analysis, showed significantly partial pressure oxygen arterial/alveolar tension ratio alveolar/arterial difference Conclusion administration partly inhibits release proinflammatory cytokines, affording protection against

Tranexamic Acid Reduces Blood Loss and Transfusion in Patients Undergoing Total Knee Arthroplasty without Tourniquet: A Prospective Randomized Controlled Trial

INTRODUCTION Blood loss during and after total knee arthroplasty (TKA) can lead to substantial morbidity and the need for blood transfusions. There are several methods to minimize blood loss and to decrease transfusion rates in patients undergoing TKA. Tranexamic acid is an antifibrinolytic agent with known efficacy for achieving these goals. Currently, many surgeons are performing TKA without ...

Effects of Tourniquet in Total Knee Arthroplasty

Tourniquet has long been used in TKA (Total Knee Arthroplasty) as the other orthopedic procedures for decreasing operation time and blood loss, providing better visualization of anatomic structures. While having much more advantages, these devices have a potential of serious complications, so they must be used with adequate knowledge and attention. Paralysis, compression neuropathy, compartment...

Intra-articular versus Intravenous Tranexamic Acid in Total Knee Arthroplasty: A Randomized Clinical Trial

Background: Total knee arthroplasty (TKA) can cause excessive blood loss requiring allogenic transfusions.Tranexamic acid (TXA) has been increasingly used for lowering blood loss. The present study aimed to comparethe efficacy of intravenous (IV) and intra-articular (IA) administrations of TXA in TKA patients who receive aspirin aschemoprophylaxis and uses no drain post-operat...
